Introduction: Iran’s Health Transformation Plan has been implemented since 2014 with the aims of promoting equity in access to health services, improving service quality, and reducing patient costs. Despite significant progress, the plan faces challenges in some regions. Therefore, this study aimed to investigate the challenges and opportunities of implementing the Health Transformation Plan in health and medical centers of Paveh city using the CIPP evaluation model.
Methods: This qualitative study employed a content analysis approach structured around the CIPP model (Context, Input, Process, Product). Data were collected through semi-structured interviews and focus group discussions with 21 purposively selected managers, physicians, and nurses from health centers in Paveh county. The collected data were analyzed accordingly.
Results: The findings revealed that inadequate physical space, insufficient equipment, and inappropriate management structures are among the main barriers to implementation. Specialized human resources and adequate training were lacking, while operational processes suffered from coordination problems and high workloads. Although patient access to services increased, overall service quality and patient satisfaction declined due to implementation-related issues.
Conclusion: The Health Transformation Plan positively impacted service accessibility in Paveh county; however, structural and human resource limitations hindered full achievement of quality objectives. Key strategies to enhance the effectiveness and sustainability of the plan include infrastructure development, strengthening human resources, improving training, and advancing process management. While the findings are generalizable to similar areas, broader studies are needed for large-scale application.
