fa شناسایی و اولویت‌بندی رویکردهای کنترل هزینه بیمارستان‌های ایران از دیدگاه نظام سلامت تخصصي Special پژوهشي Research <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:tahoma;"><strong>مقدمه</strong><strong>:</strong> تلاش برای محدود کردن هزینه‌های بیمارستانی یکی از بزرگ‌ترین چالش‌های امروز نظام سلامت ایران است. در این مطالعه تلاش شد از منظر نظام سلامت، عوامل کلیدی تأثیرگذار بر هزینه‌های بیمارستانی و رویکردهای کنترل هزینه‌های بیمارستانی شناسایی و راهکارهای کنترل هزینه متناسب اولویت‌بندی شوند.<br> <strong>روش بررسی: </strong>مطالعه حاضر، در سه مرحله انجام شد. در مرحله اول با مطالعات کتابخانه‌ای و بسط فرمول هزینه کل برای یک بیمارستان از منظر نظام سلامت، عوامل کلیدی مؤثر بر هزینه‌های بیمارستان&rlm; شناسایی شد. در مرحله دوم، رویکردهای کنترل هزینه، بسته‌های مداخله‌&rlm;ای و معیارهای اولویت‌بندی آنها در یک پانل تخصصی با حضور ۴ تن از استادان دانشکده مدیریت و اطلاع‌رسانی پزشکی شناسایی شدند. در مرحله سوم ماتریس امتیازدهی راهکارهای کنترل هزینه توسط ۲۱ نفر از مدیران ارشد بیمارستان&rlm;ها تکمیل و اولویت‌بندی شدند.<strong><span dir="LTR"></span></strong><br> <strong>یافته‌ها:</strong> با بسط فرمول تابع هزینه کل، در بخش قیمت (<span dir="LTR">P</span>) سه عامل فناوری، بهره‌وری، تورم و در بخش تعداد (<span dir="LTR">Q</span>)، سه عامل جمعیت تحت پوشش،‌ میزان بهره‌مندی و بیش‌بهری و اتلاف به‌عنوان عوامل کلیدی شناسایی شدند. برای این شش عامل، ۱۷ مولفه شناسایی شد که از این بین، ۹ مولفه قابل مداخله تشخیص داده شدند. سپس بر اساس نظر اعضای پانل تخصصی برای این مولفه‌ها، ۱۰ بسته مداخله‌ای تعریف شد. این بسته‌ها با پنج معیار &laquo;اثربخشی، تناسب، امکان‌پذیری، زمان بازدهی و بی‌درد بودن&raquo; اولویت‌بندی شدند. در نهایت ۵ رویکرد &laquo;جلوگیری از مصرف غیر مقتضی خدمات&raquo;، &laquo;تولید خدمات بیمارستانی ارزان‌قیمت&raquo;، &laquo;کنترل هزینه‌های تخت‌های بستری&raquo;، &laquo;کنترل هزینه‌های دارو&raquo; و &laquo;کنترل هزینه‌های تجهیزات پزشکی سرمایه‌ای&raquo; به ترتیب به عنوان مهم&rlm;ترین راهکارهای کنترل هزینه انتخاب شدند.<span dir="LTR"></span><br> <strong>نتیجه‌گیری:</strong> با توجه به مشکلات عدیده نظام سلامت کشور در تأمین منابع مالی، توجه به رویکردهای کنترل هزینه از منظر نظام سلامت اهمیتی فوق‌العاده دارد. این مطالعه نشان داد می‌توان با شناسایی عوامل کلیدی ایجاد هزینه و رویکردهای کنترل هزینه، راهکارهای کنترل هزینه ویژه‌ای را تدوین کرد که ضمن بی درد بودن نسبی، با سیاست‌های بالادستی نیز هماهنگ باشند.</span></span></div> <div style="text-align: justify;"><strong>Introduction: </strong>Trying to contain hospital costs is one of the biggest challenges facing the Iranian health system. In this study, we attempted to identify the key factors affecting hospital costs, hospital cost containment approaches, and to tailor intervention packages accordingly.<br> <strong>Methods:</strong> This study was conducted in three stages. In the first stage, by searching databases and expanding the total cost formula for a hospital from the health system perspective, key factors affecting hospital costs were identified. In the second stage, cost control approaches, intervention packages, and their prioritization criteria were identified in a panel of experts with the participation of 4 faculty members of the School of Management and Medical Informatics. In the third stage, the matrix scoring for intervention packages was completed and prioritized by 21 hospital managers.<br> <strong>Results:</strong> By expanding the formula of the total cost (C=P&times;Q), three factors of technology, productivity, and inflation in the Price field (P), and three factors of covered population, Utilization and overuse in the Quantity field (Q) were identified. For these key factors, 17 components were identified, which 9 components were intervenable. Then, based on expert panel members&#39; opinion, 10 intervention packages were defined. These packages were prioritized by five criteria including: &quot;effectiveness, appropriateness, feasibility, efficiency, and painlessness&quot;. Finally, the five most important intervention packages were selected as: &quot;prevention of inappropriate use of services&quot;, &quot;production of low-cost hospital services&quot;, &quot;reducing inpatient costs&quot;, &quot; containing drug costs&quot; and &quot; containing capital medical equipment costs&quot; respectively.<br> <strong>Conclusions:</strong> Considering financing problems of the Iran health system, attention to cost containment approaches is very important especially from the health system perspective.
