fa اتلاف منابع بیمارستانی طرح تحول سلامت و راهکارهای کنترل آن: موضوع تعرفه خدمات تشخیصی درمانی Hospital Resource Was tages in Health Transformation Plan (HTP) and Cos t Containment Approaches: Diagnos tic and Therapeutic Services Tariff تخصصي Special پژوهشي Research <div style="text-align: justify;">مقدمه: تعرفه گذاری صحیح خدمات سلامت، افزون بر افزایش انگیزه ارائه‌کنندگان خدمات سلامت برای ارائه خدمات اثربخش و کارآمد، از اتلاف منابع نیز می‌کاهد. این پژوهش با هدف شناسایی اتلاف منابع بیمارستانی (بعد از انجام بازنگری ارزش نسبی و تغییر در تعرفه‌ها و ایجاد نظام کنترلی برای دریافت نکردن حق‌الزحمه افزون بر تعرفه های مصوب توسط پزشکان) و ارائه راهکار در سال 1397 در شهر اصفهان انجام شد. روش بررسی: این مقاله از نوع کیفی و تحلیل محتواست. به‌منظور شناسایی اتلاف منابع بیمارستانی و راهکارهای کاهش هزینه، سایت‌های Irandoc، Google Scholar، PubMed، SID و Magiran با کلیدواژه‌های اتلاف، تعرفه، هزینه و کاهش هزینه، طرح تحول سلامت جستجو و مقالات مرتبط استخراج شد. برای تأیید و دسته‌بندی اتلاف‌ها از تکنیک گروه اسمی و برای تأیید راهکارها از بحث گروهی متمرکز با مشارکت 33 نفر از مدیران و کارشناسان ارشد بیمارستان‌ها، بیمه‌ها و اساتید دانشگاه علوم پزشکی اصفهان استفاده شد. یافته ها: مهم‌ترین نقاط اتلاف شناسایی شده در حوزه تعرفه خدمات تشخیصی و درمانی شامل نابسامانی در سطوح درآمدی برخی متخصصان، متعادل نبودن درآمد گروه‌های مختلف تخصصی، متعادل نبودن درآمد پزشکان و سایر کارکنان درمانی و تقاضای القایی بود که موجب اتلاف منابع مالی طرح تحول می‌شد و مهم‌ترین راهکارهای کاهش اتلاف شامل بازنگری تعرفه، بازآرایی ساختارهای مالی بخش سلامت و تقویت قیمت‌گذاری دقیق، مبارزه با تقاضای القایی، ترویج پزشک خانواده و نظام ارجاع و کشف سوءاستفاده و تخلف بود. نتیجه‌گیری: طرح تحول و هر طرح اصلاحی دیگری در نظام سلامت، هزینه‌هایی به همراه خواهد داشت که اگر اتلاف منابع حاصله شناسایی و کنترل نشود، موفقیت طرح به مخاطره می‌افتد.</div> Introduction: Proper tariff setting of health services in addition to reduction the health sector resources wastages, increases the healthcare providers’ incentives in delivering effective services. The purpose of this study was to identify the waste points of HTP hospital resources, and to offer some solutions in medical tariffs setting. Methods: This is an original applied research conducted employing a content analysis method. In order to identify hospital resources wastages and cost control approaches, PubMed, Irandoc, SID, Google Scholar and Magiran databases were searched with keywords wastage- tariff- Health Transformation- Cost and Cost Control Plan. Related articles were extracted. The nominal group technique was used to classify the wastes, and a focus group discussion with 33 participatants of senior managers and experts of hospitals, insurance organizations and academic members of Isfahan University of Medical Sciences was used to confirm the solutions. Results: The mos t important wastages caused by diagnostic and Therapeutic services tariffs were disparities in the income levels of some specialists, imbalances in the income of various specialist groups, imbalances in physicians’ earnings versus other medical personnel’s, and induced demand. The most important cost containment strategies included tariff review, reconstructing the health sector financial structures, and reinforcing accurate pricing, reducing induced demands, implementing family physician and referral systems, and tracing health services abuse.
