Introduction: Heart failure and cardiovascular diseases pose significant health challenges in Iran and beyond, straining healthcare systems and impacting patients' quality of life due to costs and access issues. Understanding the management challenges of these conditions is essential for improving health outcomes.
Methods: This study explores the challenges of heart failure in Iran by analyzing data from various sources, including the Medical Expenditure Panel Survey (MEPS) and the National Health Interview Survey (NHIS). It also assesses the economic burden of cardiovascular diseases using data from the Global Burden of Disease (GBD) 2019, offering a comprehensive overview of heart failure and its impact on healthcare delivery.
Results: Findings indicate that only 31% of patients with heart failure receive adequate access to cardiovascular medications. Both under-supply and over-supply of medications are linked to increased hospitalizations and higher healthcare costs. Financial barriers significantly impact access to care, particularly for low-income patients and those without insurance. Furthermore, demographic differences, such as race and income, complicate these challenges, highlighting the need for targeted interventions.
Conclusion: Effective management of medication supply and reducing financial barriers are crucial for improving health outcomes in heart disease patients. Targeted policies and strategies can enhance access to care, boost medication adherence, and lower healthcare costs. Future research should focus on diverse populations and integrate clinical data to create tailored interventions that address the specific challenges faced by cardiovascular patients, improving care quality and reducing economic burdens.

Introduction: Examining the situation of Iran's pension funds shows that these funds have faced important financial challenges in the last two decades and have somehow reached the unstable state of the pension system. This issue has seriously questioned the ability of the funds to meet their obligations for the benefit of the beneficiaries. One of the tools for measuring the balance of resources and expenses and the stability of pension funds is the support ratio. The purpose of this study is to investigate this ratio in pension funds.

Methods: The present study is a descriptive-analytical study through secondary analysis of organizational registration data. The data used were obtained from statistical and information sources of pension funds and the Statistics and Information Office of the Ministry of Cooperatives, Labor and Social Welfare.

Results: It is estimated that the support ratio in pension funds in the Social Security Organization will decrease from 5.45 in 2017 to 0.52 in 2028. This downward trend in the support ratio will also be observed in the national pension fund, that is, it will decrease from 0.95 in 2017 to 0.68 in 2028.

Conclusion: The results of this study show that the financial crisis of the funds under study will intensify during the Seventh Plan. This crisis is important in two aspects. First, the population will be covered by a high level of coverage, and second, the financial crisis in them will be transmitted to the government quickly and intensely.

Introduction: Examining factors affecting health costs in the provinces can provide a new perspective for planners and experts in social and health economics. Therefore, it is necessary to investigate the factors affecting health costs in Iran's provinces, especially (renewable energy and fossil fuels) and using spatial econometrics and calculating spatial spillovers in the provinces.
Methods: The factors affecting health costs in Iran's provinces, especially renewable energy and
non-renewable energy (fossil fuels), are investigated using spatial econometrics and calculation of spatial spillovers in 31 provinces of Iran in the period 2011-2022.
Results: The effect of renewable energy consumption on health costs is negative and significant, and the effects of fossil energy and CO2 emissions on health costs are positive and significant. Elasticity of consumption of renewable energy, fossil energy and CO2 emission are inelastic. The increase of renewable energies and the reduction of carbon emissions lead to the reduction of health costs in the provinces of Iran. Also, the spatial effect of the population over 65 years to the total population on health costs is positive and significant and is inelastic.
Conclusion: Reforming the energy price, encouraging the use of renewable energy, creating a culture to reform the energy consumption pattern are also other measures that can help reduce fossil energy consumption and, as a result, reduce CO2 emissions. Financial incentives should be considered for provinces and economic activities with high renewable energy consumption and low CO2 emissions, and taxes and fines for provinces with high CO2 emissions. For companies to act in order to reduce CO2 emission and reduce pollution.

Introduction: Given that transformational leaders are key elements in developing an organization's digital culture, they must be able to master the types of skills needed to guide followers to work in a digital environment. However, few studies have been conducted on digital transformational leadership, which indicates a theoretical gap in this area. Therefore, the purpose of this article was to design a transformational leadership model in the digital age in insurance organizations.
Methods: The research method was a mixed exploratory approach. In the qualitative section, based on thematic analysis, the components of the transformational leadership model in the digital age were identified. The data collection tool in this section was an interview with a group of experts. In the quantitative section, a transformational leadership model in the digital age was designed. Data analysis in the quantitative section was based on confirmatory factor analysis and interpretive structural modeling. Data collection in the quantitative section was based on two questionnaires.
Results: The statistical population in the factor analysis section was all managers and experts of the Health Insurance Organization with a sample of 175 people, and in the interpretive structural modeling section, the group of experts in the qualitative section. The research findings in the qualitative section showed seven main themes and thirty-four sub-themes.
Conclusion: The results of confirmatory factor analysis indicate the suitability of each of the confirmatory models. Also, the results of interpretive structural modeling show that the developed model has five levels, and the components of each row have a direct and indirect effect on other levels.

Introduction: In Iran, pension funds are defined benefit funds and their sustainability is important. However, their financial sustainability and how they are evaluated have always been criticized and studied. The welfare and social security system seeks to provide and improve the status of individuals in old age. Today, insurance is considered a key component and determinant of social security in every society. Therefore, this study addresses the improvement and development of the social security insurance acceptance model using the interpretive structural modeling technique.
Methods: This study is a cross-sectional survey study in terms of its applied-developmental purpose and in terms of the method and time period of data collection. The data collection tools were semi-structured interviews and a researcher-made questionnaire, and the reliability of the questionnaire was assessed as desirable using Cronbach's alpha and composite reliability. The qualitative part participants include theoretical experts (management professors) and empirical experts (experienced managers of the Social Security Pension Fund) in the number of 20 people. The statistical population of the quantitative part includes 384 people who are compulsory and self-employed insured in social security in Tehran. To identify the dimensions and components, the qualitative content analysis method was used, the structural-interpretive modeling method was used to determine the relationships between the components, and the partial least squares method was used to validate the model.
Results: Service marketing strategy, financial literacy of users, and physical equipment and facilities affect trustworthiness and responsiveness. Trustworthiness and responsiveness affect the improvement of customer experience and lead to customer participation, customer loyalty, and customer satisfaction. Through customer participation, it is ultimately possible to achieve the acceptance of pension funds.
Conclusion: By focusing on confirming all research hypotheses, it can be acknowledged that all components of the model have a positive and significant effect on achieving the acceptance of social security insurance.

Introduction: Failure to remove a Double-J stent on time and its retention will result in many complications for the patient. In this study, we intend to examine two approaches to follow-up and follow-up of patients after urological surgeries, considering the importance of timely follow-up and removal of Double-J stents and the importance of patient follow-up.
Methods: This retrospective study was conducted at Ziaeian hospital. A total of 573 patients underwent Double-J stent implantation over a period of 10 years and in two 5-year periods, of which 8 patients were eligible for the title of forgotten Double-J stent.
Results: During the 5-year period, the number of Double-J stent implantation cases at Ziaeian hospital was 224, of which 7 cases were hospitalized due to failure to continue follow-up treatment and as a result, failure to remove the stent. Between 2019 and 2023, the number of Double-J stent implantations was 349, of which 3 patients were called for follow-up treatment and removal of the Double-J stent using a telephone follow-up system.
Conclusion: This study attempts to present the role of designing low-cost follow-up systems for continuing treatment and postoperative examinations in a transparent manner so that physicians and other medical staff realize the importance of providing and designing a coordinated, simplified, operational, and inexpensive system for coherent patient follow-up.
