Introduction: With the growing population of migrants in Iran’s host provinces, including Yazd, providing health services to insured foreign nationals has imposed a substantial financial burden on insurance resources. This study aimed to estimate the financial burden of health services provided to insured foreign nationals in Yazd province.
Methods: This cross-sectional descriptive-analytical study was conducted in 2024. The study population included all foreign nationals covered by health insurance in Yazd who utilized medical services during that year. Data were extracted from the HIS system and the Iran Health Insurance Organization database, cleaned, and analyzed using SPSS version 25.
Results: In 2024, a total of 4,493 hospitalizations of foreign nationals were recorded, of which 53.8% were female and 46.2% male. The total cost of inpatient services amounted to 160.7 billion IRR, with the Health Insurance Organization covering 90.5% of this amount. Physician services represented the largest share of costs (66%). The recovery discharge rate was 99.2%, while in-hospital mortality accounted for 0.8%. Children under five and middle-aged women were the groups with the highest financial burden.
Conclusion: The financial burden of health services for foreign nationals in Yazd is considerable. Revising financing policies, strengthening primary care, targeting insurance coverage, and securing international support are essential to improve the financial sustainability of the Health Insurance Organization.
