According to the law of the Seventh Social, Economic, Political, and Cultural Development Program of the country, the operator system in Iran's e-health has been assigned to the Ministry of Health and Medical Education. According to it, the Ministry of Health and Medical Education has the role of regulator and operators, both public and private, obtain licenses to operate in order to implement e-health services. In other words, this system is a model in which policy-making and supervision (regulatory) are separated from the role of implementation and service delivery (operatory). The Statistics and Information Technology Office of the Ministry of Health, a subsidiary of the Deputy for Development and Resource Management, is responsible for fulfilling this task. Some efforts have been made. It seems that the expected results have not been achieved as expected. Examining global experiences and leading and successful countries can help accelerate the realization of the program's goals. Global studies have highlighted issues such as the nature and independence of the regulator, the sovereignty of operators, their ownership and revenue models, the transparency of administrative and financial information, along with strong standards and laws in the field of implementation and compliance of executive bodies, operators, and companies with the regulatory standards, and the existence of demand systems. In Iran, there are shortcomings in these areas that need to be corrected.
